Protective Medical Decisions Document (PMDD) – Durable Power of Attorney for Health Care
The Patients Rights Council’s Durable Power of Attorney for Health Care is called the Protective Medical Decisions Document (PMDD). In it, the signer designates an agent to make health care decisions in the event that the signer is unable to do so. The document expressly defines and prohibits euthanasia. Available only in sets. Each PMDD set includes 3 documents, instructions, a checklist and a wallet card.
State-specific versions for as well as documents that meet the statutory requirements in all other states are available.
